MTP 03/02/2021 OTOW VACCINATION

 COVID Vaccines are coming to OTOW!

 

To make your COVID 19 vaccination appointment, call 352–479–3105 Tuesday through Thursday, 9 am to 4:00 pm.  If you receive a busy signal, please keep trying.  Thank you for your patience as our team works to set appointments for those wishing to receive the vaccine.

 

Vaccines will be given over a three day period -- March 5, 6, and 7.  Information regarding the appointment and day of vaccine is provided below—strict guidelines are enforced: 

 

·         You must have an appointment to get a vaccine or you will be turned away. 

·         The vaccine is supplied by Pfizer.

·         The Incident Management Team (IMT), per State guidelines, is not able to administer vaccine to anyone that is prescribed an EpiPen, nor is able to administer vaccine to anyone that is under 65 years old.

·         Per DOH guidelines, if you are sick with COVID-19, do not get the vaccine.

·         If you have had a first dose elsewhere, do not book an appointment - Your second dose has already been allocated, and may go to waste if you do not use it.

·         If you have a scheduled appointment through DOH, please do not book an appointment here.

·         Vaccines will be given at the Circle Square Cultural Center Portico Vehicle Drive Through located 8395 SW 80th St., Ocala, FL  34481, March 5, 6 ad 7 by appointment only.  Follow the signs

·         You will be required to have a photo ID to verify identity and age at time of visit. Masks are required to be worn at all times during your visit.

·         PLEASE DO NOT ARRIVE MORE THAN 15 MINUTES BEFORE YOUR SCHEDULED APPOINTMENT OR YOU WILL BE TURNED AWAY.  

·         GOLF CARTS ARE NOT ALLOWED.

·         To expedite the vaccination process, please print out the following form and bring with you. Please fill out to the best of your ability.
